Physical Sign in Form

Always have on the desk (folder), so we can cross reference with App Sign in + plus it is needed for the Facility Owner to keep the Physical Count

you sign the child in with a circle around M T W TH F (day of the week) parents will initial out.

*Very Important, Authorized pick up names are in the App or emailmeform.
Make sure you Ask for ID on the very first time you see someone other than Mom or Dad (whichever one you have not seen)

Electronic Accident/Incident Form

Once Filled out - fill out the info presented to you; once submitted.
this will be sent to the parent via email.
please text parent to confirm they received.

Behavior Forms

Campers (1st-3rd) Behavior E-Form

Once filled out - a copy will be sent to parents via email;
**Please make sure Admin team knows and/or Aware of the Childs Situation. Last Warning is Suspended from the Camp the remaining time of Camp

Staff 1st-2nd) Warning E-Form

This will be sent to Admin and the Staff that this is pertaining to.
